Vetting Form

If you are 18 years old and older, you need to fill in Form NVB 1 only. 

If you are 15 years old and under you do not need to complete vetting.

If you are 16 or 17 years old, you need to fill in both forms below the NVB 1 and NVB 3.

All vetting forms must be handed back to the school office and a copy of ID and proof of address will be photocopied in school.
